Shift to higher-paying codes

Doctors bill Medicare for 200 million visits every year using codes to reflect the complexity of the treatment and the time required. Over the past decade, doctors have increasingly used higher-paying codes while cutting back on lower-paying codes, according to a year-long investigation by the Center for Public Integrity.
